Referring to the CARE Team - Submit a Concern

H​ow do you make a referral to the CARE Team?

Complete and submit the Student Care Form.
A member of the CARE Team will review this information Monday-Friday within 24 business hours.

Note: The Student Care Form is not for emergency situations. In case of an emergency call 911 on a campus phone or 972-860-4290 from a cell phone.

How do you know when to refer someone to the CARE Team?

You should refer someone to the CARE Team anytime you are concerned about the safety or well-being of a student, faculty or staff member or the safety of the college campus. All referrals are treated with the highest level of confidentiality.

The CARE Team will do everything possible to protect your identity. Submitting anonymous reports makes it more difficult for the team to take action. If you must remain anonymous, please provide as much information as you possibly can.

What to refer:

If you become aware of one or more individuals displaying the following behaviors or concerns, please refer them to the CARE Team.

  • Changes in demeanor
  • Drastic changes to grooming or dress
  • Appears sad or depressed
  • Suicidal remarks or attempts
  • Emotional responses are unusual or exaggerated
  • Exhibits highly disturbing behaviors
  • Disrupts class
  • Submits disturbing written material
  • Making threats
  • Makes references to guns

Please also refer those individuals for whom you have general concerns. The CARE Team will connect with the individual and provide support.

How does the CARE Team work?

Once a referral is received, the CARE Team reviews the referral, gathers any collateral information, determines the level of concern and the intensity of the response needed to address the concerns referred and provides support to the individual.

The CARE Team meets on a regular basis. Someone from the CARE Team will be identified to reach out to the individual who has been referred and provide additional screening to identify what services, support​​ or resources would be beneficial. A plan will then be developed to address the individual’s concerns.
