Health Monitoring and Mobile App

​​Our campuses are currently closed to the general public at this time. Most classes will be held online through the Spring 2021 semester.

​Accurate self-reporting is a critical aspect of health and safety. Employees who work on campus are required to self-report on​ a daily basis prior to entering the building. It must be completed within two hours of entering the building to ensure the most accurate information.

Mobile App

To help make the pre-screening process as seamless and convenient as possible, Dallas College has contracted with Appian for a mobile app that employees can use. The app can be accessed as follows:

How to Use the Appian App

Open the Appian App.

The first time you use the app, you will be prompted to setup your account and enter the Dallas College Appian website.

After you enter the website information, sign-in to the Appian mobile app by selecting "I'm an employee or student."​

  1. You will use your Dallas College single sign-in information.
    • As a reminder, employees will need to authenticate your login with your multi-factor authentication method.
  2. Complete Profile information and/or Update information if needed
    1. Select Department (Ex. HR, IT, Student, etc.)
    2. Employees need to choose Office-Based and Students need to choose Campus-Based for Employee/Student Type
    3. Choose the location where you will be returning (Example – Employee, choose RLC – Employee, Students choose RLC – Student, etc.)
    4. Enter Current Residence or Home Address
    5. Select “In Office” for Employee and “On-Campus” for Student for Current Work/School Status
    6. Answer the Yes/No radio questions
  3. Click “Continue”
  4. Click the acknowledgment box that attests the information is true and accurate
  5. Click “Submit”
  6. After submitting the questionnaire, the system will bring the employee/student to a landing page indicating their eligibility to return work.
    • If the employee/student is not eligible the system will tell the employee to continue to work from home.
    • If the employee/student is eligible the system will provide the individual with a pass that grants access for that day. The employee/student needs to show the pass to gain access to the campus.

The questionnaire should be completed daily no more than two hours before the accessing the campus.

Instructions for Changing the Language Preference

Visit the Dallas College Appian app website at and sign in by clicking “I am an employee, contractor or student” if you are not already signed in.

Once you are signed in, click on the profile image in the upper right corner.

In the box that pops up, click “Settings.”

Click on the “Language” drop-down menu and select your preferred language. Click the blue “save changes” button in the bottom right of the settings box, and then refresh the page by clicking the refresh button on your Internet browser.

Other Access

If you are unable to use the mobile app, please contact your local HR representative for an alternate way to fill out the questionnaire.

Temperature Checks

The self-reporting includes having employees take their temperatures within two hours of coming to work.

Additional Information

If warranted, in accordance with guidance issued by public health authorities and local, state, and federal government orders, you may be asked to self-quarantine for a temporary period.

Any data collected will be considered confidential information and used solely by Dallas College in its efforts to reduce the risk of COVID-19 transmission in the workplace and/or implement an efficient and effective response to the COVID-19 pandemic. Information provided will not be disclosed, except as necessary to apply or implement the Dallas College's workplace safety and COVID-19 response protocols, and other related policies and procedures, or as required by law. Information will be stored in accordance with Dallas College retention policies.

This questionnaire is voluntary; you may decline to complete all or part of the questionnaire. If you decline to respond to the questionnaire, you will be asked to stay home or leave the workplace temporarily as a workplace health and safety measure, and the Dallas College will take appropriate additional workplace health and safety measures as needed to reduce the risk of COVID- 19 transmission.
