3. Home telephone number
4. E-mail
5. Field of study
6. Photograph
7. Date and place of birth
8. Dates of attendance
9. Enrollment status, ( i.e., full or part time, graduate or undergrad )
10. Degrees, certificates and other honors and awards received
11. Type of award received ( i.e., academic, technical, Tech-Prep, or Continuing Ed. )
12. Participation in officially recognized activities
13. Weight and height of members of athletic teams
14. Student classification
15. Name of the most recent previous institution attended
