and the employees pay 50% of the pre-taxed premium.

Employee may automatically be enrolled in
the Medical Plan on the first day of the month after 90 calendar
days of active employment. Dependents must be enrolled by the employee by the 91st
day of employment, otherwise evidence of insurability may
be required. |

Same as comprehensive medical. |
Employee may apply for additional state funds
at any time during the plan year to pay for child(ren). |
Employee may add
SKIP approved child(ren) to medical insurance at a reduced

Indemnity Plan – May see any Dentist;
eligible dental work covered. Benefits increase during
the first 3 years of enrollment. DMO – Must live in service area and select from
the list of approved Providers. Eligible dental work covered
with employee paying applicable co-payments. |

Optional Group Term Insurance

For 2 times annual base salary within the 30 days of hire.
May apply through evidence of insurability for up to 4
times annual base salary.
Otherwise proof of insurability is required.

Short-Term Disability

Your maximum monthly income benefit will be 60% of your insured monthly salary (maximum $3,000) or $1,800, whichever is less. Your minimum monthly income benefit will be 10% of your insured monthly salary (if you are receiving worker's compensation, a disability retirement benefit, or other group disability benefits).
Benefits are payable up to five months after you have been certified as totally disabled by an approved practitioner, used all of your sick leave, extended sick leave, sick leave pool, or completed the waiting period of 30 consecutive days, whichever is greater, and have been approved by Fort Dearborn Life.

Long-Term Disability

Your maximum monthly income benefit will be 60% of your insured monthly salary (maximum $10,000) or $6,000, whichever is less. Your minimum monthly income benefit will be 10% of your insured monthly salary for up to 12 months (if you are receiving Social Security disability, worker's compensation, a disability retirement benefit, or other group disability benefits).
Benefits are payable to you after you have been certified as period totally disabled by an approved practitioner, used all of your sick leave, extended sick leave, sick leave pool, or completed the waiting period of 90 consecutive days, whichever is greater, and have been approved by Fort Dearborn Life. Benefits are payable up to the maximum benefit period, which depends on your age when you become totally disabled:
Under 60 - Payable to age 65
60-64 - Payable for 60 months
65-69 - Payable to age 70, or 12 months, whichever is greater
70 and over- Payable for 12 months

Under IRS Guidelines, allows employee's pre-tax

Accelerated Life

An active employee on approved leave without
pay, with a qualified illness may apply for 50% of their
Optional Term Life Insurance, up to a maximum of $100,000. |
COBRA

Under COBRA an employee may continue medical
and/or dental for a maximum of 18 months due to termination,
other guidelines apply for employees and dependent(s).
