Request to Lift Financial Aid Suspension (PDF - 44KB)
Use this form to request special consideration when you have been placed on suspension from financial aid for failure to comply with the Satisfactory Academic Progress (SAP) policy.

Financial Aid Verification Form (2008-09) (PDF - 46KB)
If you are selected by the U.S. Department of Education for a process called “verification,” you must complete and submit this form to the Financial Aid Office with signed copies of your and/or your parents’ or spouse’s federal income tax return. If you or your parents will not file federal income tax returns, you and/or your parents must submit copies of all W-2 forms or other income documentation.

Special Circumstances Application (2008-09) (PDF - 44KB)
A family’s 2007 income is used to assess financial need for the 2008-09 school year. If your family’s anticipated 2008 income is at least 25 percent lower than actual 2007 earnings, complete this form to request that the 2008 anticipated income be used to assess financial need.

Dependency Status Change Request (2008-09) (PDF - 43KB)
If you are eligible, you may complete and submit this form to request a change in dependency status for federal student aid purposes.

Certification of Nonfiler/Low Income Verification Form (2008-09) (PDF - 22KB)
Use this form to report income information if you and/or your parents or spouse did not file federal income tax or reported an income of less than $5,200 on your Free Application for Federal Student Aid (FAFSA).

Affirmation of Loan Responsibility for Borrowers With Disabilities (PDF - 15KB)
Federal student loan applicants who have had previous federal loans discharged because of a disabling condition must complete and submit this form to the Financial Aid Office with a physician’s statement certifying the applicant’s ability to engage in substantial gainful activity.

Financial Aid Pre-Application Worksheet (2008-09) (PDF - 261KB)
If you are a Texas resident but are not eligible for federal student aid because you are not a U.S. citizen, you may use this form to apply for Texas state financial aid. Complete this form and submit it with supporting income verification documents to your Financial Aid Office.
