Summer I 2008 Financial Aid Application (PDF - 125KB)
Summer financial aid eligibility is limited to students who plan to attend a DCCCD college the following fall semester and/or to current DCCCD students if they have PELL funds available for the current award year. Transient students (coming from a school outside DCCCD and not transferring to DCCCD) are not eligible to apply for financial aid. Applicants must have completed a Free Application for Student Aid (FAFSA) online, and a 2007-2008 Student Aid Report (SAR) must be in the DCCCD Financial Aid system.

Request to Lift Financial Aid Suspension (PDF - 16.1KB)
Use this form to request special consideration when you have been placed on suspension from financial aid for failure to comply with the Satisfactory Academic Progress (SAP) policy.

Financial Aid Verification Form (2007-08) (PDF - 29.1KB)
If you are selected by the Federal Department of Education for a process called “verification,” you must complete and submit this form to the Financial Aid Office with signed copies of your and/or your parents’ or spouse’s federal income tax return. Copies of all W2 forms must be submitted as well.

Special Circumstances Application (2007-08) (PDF - 54.2KB)
A family’s 2006 income is used to assess financial need for the 2007-08 school year. If your family’s anticipated 2007 income is at least 25 percent lower than actual 2006 earnings, complete this form to request that the 2007 anticipated income be used to assess financial need.

Dependency Status Change Request (2007-08) (PDF - 19KB)
If you are eligible, you may complete and submit this form to request a change in dependency status for federal student aid purposes.

Certification of Nonfiler/Low Income Verification Form (2007-08) (PDF - 8.88KB)
Use this form to report income information if you and/or your parents or spouse did not file federal income tax or reported an income of less than $5,200 on your Free Application for Federal Student Aid (FAFSA).

Affirmation of Loan Responsibility for Borrowers With Disabilities (PDF - 70.5KB)
Federal student loan applicants who have had previous federal loans discharged because of a disabling condition must complete and submit this form to the Financial Aid Office with a physician’s statement certifying the applicant’s ability to engage in substantial gainful activity.

Financial Aid Pre-Application Worksheet (2007-08)
If you are a Texas resident but are not eligible for federal student aid because you are not a U.S. citizen, you may use this form to apply for Texas state financial aid. Complete this form and submit it with supporting income verification documents to your Financial Aid Office.
