Disability Services Accommodation Request Procedure

1. Contact the Disability Services office.

2. Submit application to the college chosen.

3. Complete paperwork provided by Disability Services office.

4. Meet with Disability Services advisor (appointment recommended)

5. Present supporting documentation of a disability or a signed release for request of documentation.

6. Hold meeting with Disability Services advisor for assessment of educational, vocational, personal and accommodation needs. (allow one hour).

7. Attend new student orientation (allow two hours) if you are a recent highschool graduate and are new to college.

8. Complete assessment testing, when appropriate, in the testing center (with accommodations if approved).

9. Complete student advising with Disability Services advisor. Accommodation needs are addressed at this time and provided dependant upon documentation and advisor assessment. Notice: Students are not required to be advised and registered through Disability Services Office but will need to apply for accommodations through the Disability Services Office.

10. Complete registration for college classes.

Special Accommodations:
If there is a need for assistive technology or a sign language interpreter please allow 2 to 4 weeks for these accommodations to be put in place.
